Since we are joining a new league, our sign ups process will be slightly different this year. ALL PARTICIPANTS MUST PROVIDE AN ORIGINAL BIRTH CERTIFICATE AND REPORT CARD WITH PASSING GRADES. A copy of the birth certificate with remain with the certified participant card and original returned to you as soon as MVL certifies our books in August.

Forms are different this year and we apologize for any inconvenience. All participants must complete:

Emergency Medical Treatment Consent form (medical insurance information required)
Player/Parent contract (signed by BOTH parent and participant)
Waiver & Release of Liability (signed by BOTH parent and participant)
Image Release (signed by parent)
Medical Clearance Form (completed by personal physician or at our physicals-date TBD)
Participation Contract (signed by BOTH parent and participant)
